Healthcare has more to gain from AI than almost any other sector. It also has more ways to get AI implementation wrong.
Clinical teams are drowning in documentation. Administrative staff are managing processes that haven't changed in fifteen years. Compliance requirements generate paperwork that consumes hours that should go to patients. The volume of structured and unstructured data in a healthcare organisation is enormous — and most of it sits in systems that don't talk to each other.
AI should be transforming this. In many organisations, it isn't. Not because AI can't work in healthcare — it demonstrably can — but because the implementation approach is wrong.
Where AI actually delivers value in healthcare operations
Clinical documentation. Clinicians spend more time documenting than doing clinical work at many organisations. The key word is "structured." AI documentation tools that work well in healthcare are not generic voice-to-text — they're systems trained on clinical language, structured to produce documentation in the right format for the specific workflow (SOAP notes, nursing assessments, discharge summaries), and integrated into the EHR rather than requiring a separate interface.
Patient intake and triage support. AI-guided intake workflows that collect structured patient information before a clinical encounter — symptoms, history, medications, allergies — both improve the quality of information available to the clinician and reduce the administrative burden of intake. Well-implemented intake AI produces structured, reviewable summaries rather than free-text form responses, which saves clinician time and reduces the risk of missed information.
Prior authorisation and insurance documentation. The prior authorisation process is a significant administrative burden for clinical teams and a source of treatment delay for patients. AI that can review a treatment plan, identify the applicable insurance requirements, pre-populate the authorisation request with relevant clinical documentation, and flag where additional information is needed reduces both the time and the error rate in this process.
Coding and billing documentation. Accurate medical coding requires detailed clinical documentation. AI that reviews clinical notes and suggests appropriate diagnostic and procedure codes — with the clinician as reviewer rather than author — reduces coding errors and the administrative time required for medical billing.
Compliance documentation and reporting. Healthcare organisations operate under significant regulatory requirements that generate substantial documentation obligations. AI that monitors for compliance gaps, generates required reports, and flags documentation deficiencies before they become audit findings is more valuable than post-hoc compliance checking.
Where AI in healthcare doesn't work yet
Autonomous clinical decision-making. AI that makes clinical decisions without clinician oversight is not ready for production use in most contexts. The liability exposure is significant, the regulatory framework is evolving, and the consequences of AI error in clinical decisions are severe. The right model is AI-assisted decision support — surfacing relevant information, flagging potential issues, suggesting options — with clinical judgment making the final call.
Replacing human communication with patients. Patient communication that involves clinical judgment, emotional support, or complex information sharing should not be fully automated. AI can support this work — drafting messages, summarising relevant information, answering routine administrative questions — but the human element in clinical communication is not a cost to be optimised away.
Unsupervised operation in high-stakes workflows. AI systems in healthcare require robust monitoring, fallback handling, and clear escalation paths for edge cases. An AI system that fails silently in a clinical workflow can cause harm. Production healthcare AI needs the same reliability engineering discipline as any other critical infrastructure.
The HIPAA and compliance reality
AI systems in healthcare operate under HIPAA and, depending on the use case and jurisdiction, FDA regulatory requirements. This creates specific engineering requirements that generic AI tools don't address.
Data handling. Protected health information (PHI) that passes through an AI system needs to meet HIPAA technical safeguards — encryption in transit and at rest, access controls, audit logging. Many off-the-shelf AI tools have not been built with these requirements in mind.
Business Associate Agreements. Any AI service that processes PHI needs to have a Business Associate Agreement with the covered entity. Not all AI vendors offer BAAs. Not all BAAs that are offered provide adequate protections.
Audit trails. Clinical AI systems need to maintain detailed audit trails — what AI output was produced, when, based on what input, and what the clinician's action was in response. This is both a regulatory requirement and a clinical safety requirement.
Model transparency. In clinical contexts, the ability to understand why an AI system produced a particular output is important for clinician trust, for error investigation, and in some contexts for regulatory compliance. Black-box AI is harder to deploy in healthcare than in less regulated sectors.
What good healthcare AI implementation looks like
The healthcare organisations that are getting genuine value from AI built it with clinical workflow as the starting point, not AI technology as the starting point.
They identified specific, high-volume administrative processes — the intake forms, the prior auth requests, the discharge summaries — and built AI systems that integrate into the clinical workflow rather than adding to it. The AI output goes directly into the EHR. The clinician reviews rather than authors. The time savings are measured in specific workflow metrics, not in general productivity claims.
They built with the compliance requirements as engineering requirements from the start — not as afterthoughts to be addressed when the tool is nearly done. HIPAA-compliant data handling, BAAs, audit trails, and appropriate model transparency were part of the architecture.
